
The
Department of Registration of Persons has requested the general public
to use the new application form bearing the number D.R.P. V/01, 07, 08
from 30,03.2016.
A press release signed by Commissioner
General of Registration of Persons said: Regulations for registration of
persons have been amended providing for using a common Application Form
in place of three separate forms that are currently being used to apply
for a new Identity Card. (Vide Gazette Extraordinary No. 1946/31 defied
22.12,2015 for further details.)
As per above amendments, the Application form D.R.P V/01, which is currently in use to apply for an identity card for the first time under Section 09 of the Registration of Persons Act, the Form D.R.P V/07, which is used to apply for a duplicate of a lost identity card under Section 16 of the Act, and the Form D.R.P V/08, which is currently in use to apply for an amended duplicate of an identity card under Section 17 of the same Act will, be withdrawn after 29.03.2016.
The public is hereby informed to use the new Application Form bearing the number D,R.P. V/01, 07, 08 with effect from 30,03.2016. This new Application Form is valid for obtaining an identity card for the first time under Section 09, applying for a duplicate of a lost identity card under Section 16 or obtaining an amended duplicate of an identity card under Section 17.
This new Application Form, may be obtained from any District Secretariat, Divisional Secretariat, Grama Niladhari, Principals, and Estate Superintendent. In addition, it may be downloaded from the department’s website www.drp.gov.lk. Application Forms obtained from any of the above sources should be duly completed and submitted to the appropriate certifying officer (Grama Niladhari, Principal, Estate Superintendent) with relevant documents for onward transmission to the Department for Registration of Persons. It is kindly requested to submit photographs of the size of 1 3/8 X 7/8 instead of the photographs of international standard (35mm X 45 mm) until photographers are trained.
